Hutchinson Medical Center to require proof of ID
HUTCHINSON, Minn. In an effort to protect patient identities and reduce health care costs, Hutchinson Medical Center is now asking for personal identification for all appointments. This requirement may be waived for patients who have visited the practice within the last year.
HMC asks that patients provide a driver’s license or other photo ID and a copy of an insurance card at the appointment. Anyone without a photo ID showing a current address will need to provide a utility bill or other correspondences proving residency. If the patient is a minor, a parent or guardian should bring this information.

After Jan. 1, 2010, patients who fail to bring this information to their appointments will be asked to reschedule until identification can be provided.
For the protection of its patients, Hutchinson Medical Center, P.A. will be alert for discrepancies in documents and patient information that suggests risk of identity theft or fraud.
